Web Log Report Request

If you wish to request reports for more than one site, fill out the form for each site. Reports contain information on number of hits, files and directories most/least accessed, paths taken through the site, top search terms, etc. Reports are generated on the first of each month in PDF format and are available via the GWUMC Control Panel. If you do not have a Control Panel account, one will be created for you when your request is processed.

* Site URL	Enter the URL for your site including domain name and path. Example: inside.gwumc.edu/webservices
* Department
* First Name
* Last Name
* Title	Enter your title.
* Email Address	Enter your @gwumc.edu, @gwu.edu, or @mfa.gwu.edu email address.
* Phone
Run Monthly	Yes, run this report every month. No, run this report one time only.
Special Requests	Use this space to enter special requests (such as custom date range or tracking specific files), or names/email addresses of additional people who should receive the report. Maximum 1000 characters.
